a randomized phase II trial of enzalutamide and talazoparib in metastatic hormone-naïve
prostate cancer
ZZ-FIRST explores the potential of treatment intensification through the combination of PARP inhibition and androgen receptor (AR) blockade in patients with high-volume metastatic hormone-sensitive prostate cancer (mHSPC).
The study evaluates whether the addition of talazoparib to enzalutamide and androgen deprivation therapy may improve clinical outcomes, while advancing the understanding of early tumor adaptation, treatment response and resistance to androgen receptor pathway inhibition through translational analyses conducted during the initial phases of treatment.

Previously investigated in more advanced disease stages, this therapeutic combination is now being explored earlier in the metastatic setting, representing a novel approach in hormone-sensitive prostate cancer and reflecting MEDSIR’s continued commitment to investigator-driven research designed to address meaningful clinical questions in oncology.

The study explores whether this approach can serve as an optimized endocrine partner for ribociclib in patients heavily pre-treated with endocrine therapy in earlier settings.

By combining an oral SERD (elacestrant) with mTOR inhibition (everolimus), the study explores a potential approach to restore endocrine sensitivity and improve outcomes in a population with limited post–CDK4/6 inhibitor treatment options.
